Member Resources: Death Benefits

DC37 is the parent Union of Local 3621.  They provide a $10.000 death benefit to the beneficiary on file or the estate if none was selected. Call the Death Claim Unit to file a claim, 212-815-1373 or 1374. 
Annuity Fund:
Every member has an annuity fund at DC37.  Please call 212-815-1888 to file a claim for this benefit.  It is payable to the selected beneficiary, or to the estate of the deceased of none was selected.
FDNY: 9 MetroTech Center, Brooklyn, NY 11201
If the member passes in the line of duty, the FDNY pays to the members’ beneficiary a sum of $25,000.00.  If they do not have a beneficiary, the sum is paid to the estate of the member.  You can contact the FDNY personnel Office to file this claim, 718-999-2165.  The beneficiary is also entitled to the member’s vacation, sick and Compensation Time balances as well as the last pay check.  Make sure you inquire about all of them.  Also note** If you and your family received health insurance through the member, those benefits cease upon death.  You can ask Personnel about COBRA benefits to continue them. 
PSOB: The Public Safety Officer Benefit
This benefit was created by the Government to help people provide for their families in case of a line of duty death.  The Government realized that many people were not considering dangerous jobs out of fear that something would happen to them, and their family would suffer financially.  This benefit increases periodically and is approximately $275,000.  The FDNY EMS Operations Office should file this claim, if they do not, please contact the Bereavement Officer on the front of this brochure to see if the Local can file it for you.  FDNY EMS Operations Office 718-999-2770.
NYCERS: 335 Adams Street, Brooklyn, NY 11201
This is the NYC Employees Pension Fund.  Here, you would have to file a claim for Line of Duty Death (LODD) benefits, as well as the members contributions   347-643-3000

Worker’s Compensation:
Under NYS law, if you sustain a mortal injury while at work, the state will compensate the beneficiary of the member.  You will need to contact a Worker’s Compensation Attorney to assist you in filing that claim, unless you wish to file it on your own.  If you do not have a Comp attorney, please contact the union office for a list of local representatives to assist you.
Please remember that the beneficiaries on file will receive all benefits and if none was selected by the member, the benefits are awarded to the estate of the member.  If you need assistance with the laws of NYS that govern these benefits, please seek an estate attorney.

Retired Members Passing as a result of a LOD Injury/illness.
If the member passes as a result of a Line of Duty Injury/Illness, there may be other benefits to apply for.  NYCERS must be notified for the death option selected, DC37 offers $1000.00 to the beneficiary, a Worker’s Comp Death claim needs to be filed, and a PSOB claim made by the Union.
